How to File a Claim |
In the event of an interscholastic sports or special school activity injury:
1) Report the injury to the Supervisor, Coach or Athletic Department office immediately after the accident and advise them that you want to file a claim report due to a school related accident. The Supervisor, Coach or the Athletic Department will fill out a claim report and forward the report to the School Insurance of Florida claim office.
2) School Insurance of Florida will mail to you a claim form to complete and details concerning the policy benefits and limitations.
3) When you receive the claim filing information from School Insurance of Florida follow the claim filing instructions. Mail the completed form, copies of itemized medical bills and evidence of payments received from your primary family insurance to School Insurance of Florida. Our address will be printed on the claim form. Please be sure to forward the completed claim form to School Insurance of Florida no later than 90 days after the date of accident to be eligible for policy benefits.
DO NOT CALL THE SCHOOL ABOUT THE STATUS OF A CLAIM PAYMENT OR TO ASK QUESTIONS ABOUT COVERAGE! The school does not process claims or keep records regarding claims. All claims are processed by School Insurance of Florida's claim office. Our claims office will provide you with prompt answers to claim or coverage questions.
IMPORTANT: When contacting School Insurance of Florida's office by mail, fax or email about a claim, please state the full name of your injured son or daughter and the school name where he or she attends school. We need the school name to reference the claim information for you. Generally, valid claims are processed within 7 working days after we receive your completed claim information. |
